education advocacy, supports to reduce truancy,
and aims to connect students and their families
with valuable resources and opportunities to help
them thrive.
ENHANCED FAMILY
FOSTER CARE (EFFC)
The Enhanced Family Foster Care (EFFC) model
combines Family Foster Care and Therapeutic
Family Foster Care to support children who have
experienced trauma. EFFC offers flexible, ongoing
assessments and tailored services to address the
changing needs of families. This increases stability
and fosters secure attachments between children
and caregivers, while also providing enhanced case
management and comprehensive support to families
in New York City as they work towards permanency.
HEALTH HOMES
The goal of the Health Home program is to make
sure NYS Medicaid members get the care and
services they need. A Health Home is not a physical
place; it is a network of health care and service providers chosen by Abbott House. These resources are
strategically put in place to ensure that individuals
get what they need to stay healthy. Once an individual is enrolled in a Health Home, they will have a care
manager that works directly with them to develop a
care plan. Abbott House accepts Health Home referrals on our website.
