H.E.A.R.T. PERMENANCY
RESOURCE CENTER (PRC)
The H.E.A.R.T. PRC offers support and resources in
Westchester, Orange, and Dutchess County at no
cost to post-adoptive families, legal custody and
legal guardianship families and families in informal
custody arrangements. The PRC staff works to
strengthen family bonds and welcomes adoptive,
guardianship and relative caregivers, as well as nonparent caregivers of children with no formal legal
designation. A history in foster care is not required,
and self-referrals are accepted on Abbott House’s
website.
NON-SECURE DETENTION (NSD)
Abbott House operates two Non-Secure Detention
Group Homes – one for females in Richmond Hill,
and another for males in East Elmhurst. Each home
has a 12-bed capacity and provides a therapeutic
environment to help youth with a history of juvenile
offenses to develop healthier life choices and
positive behaviors while awaiting their court dates.
Referred by the Administration for Children's
Services' Division of Youth and Family Justice,
residents attend Passages Academy at the Summit
School, run by the NYC Department of Education's
Division of Alternative School Programs.
QUALIFIED RESIDENTIAL
TREATMENT PROGRAMS (QRTP)
Abbott House Group Homes provide a safe and
supportive environment for adolescents unable to
live with their families, offering individualized care
plans, therapeutic services, educational support,
recreational activities, and life-skills development.
Recently certified as Qualified Residential
Treatment Programs (QRTPs) and accredited by
CARF International (Commission on Accreditation
of Rehabilitation Facilities) for excellence in care,
these home environments offer evidence-based,
trauma-informed care for youth aged 15-21. Each
home accommodates up to eight youth and offers
comprehensive support, including medical care,
social work services, educational assistance, and
independent living skills.
HOME AND COMMUNITYBASED SERVICES (HCBS)
Home and Community-Based Services (HCBS) is
a network of support services that help children
and youth under the age of 21 who are enrolled (or
eligible to enroll) in Medicaid, enrolled/eligible for
Health Home, and need additional care at home and/
or in the community. HCBS enables children/youth
to participate in many developmentally and culturally
appropriate services. Referrals for HCBS come from
Health Home care managers.

THERAPEUTIC FAMILY
FOSTER CARE (TFFC)
Therapeutic Family Foster Care (TFFC) offers
intensive support to children with serious emotional,
behavioral, or medical needs in a structured
home environment, while foster parents receive
specialized, trauma-informed training. Abbott House
provides this program in the Lower Hudson Valley,
ensuring safe, nurturing, and healing care for children
with specialized needs while working with their
families to achieve permanency.
